All this should be included with your submission (pasted into the box below...please make sure you include all information requested.  Font style and submission requirements should have been read prior to submitting. If in doubt- please review our submission guidelines.

Look over the form before you send it...you can make editing adjustments within this form- please don't send us a garbled mess- one long sentence with no spacing.  If it looks bad in here, it will look bad to us.  We might reject a good story because it's not properly formatted, so save us all a lot of trouble and submit properly.
